Part‑Time/Weekend Physical Therapist Case Manager – Home Health

Join 21st Century Home Health Services (21HHS), the largest home‑health agency in San Francisco, as a Part‑Time Physical Therapist Case Manager. This role requires field visits to patients’ homes to assess, treat, and coordinate care.

Pay: $100–$150 per visit. Minimum 15 visits per week on a 3‑day workweek. Mileage reimbursed at IRS standard rate. Additional $20 for missed visits, $35/hour for non‑visit work.

• Perform in‑home evaluations and develop individualized care plans.
• Coordinate with PTAs to ensure plan adherence and support patient independence.
• Assist patients with mobility and daily activities.
