Job summary

West Bay Collaborative in Rhode Island seeks a Part-Time Instructional Transition Facilitator/Job Coach to deliver Pre-Employment Transition Services and provide job coaching for adolescents and adults with disabilities in school and community settings.

You will develop and deliver curricula for students with IEP/504 plans, collaborate with teachers and counselors, monitor progress, and support learners in acquiring job tasks and workplace routines.

Qualifications

• Associates degree in human services required; bachelor’s preferred.
• Experience with adolescents, knowledge of developmental, mental health, physical, and learning disabilities.
• Excellent communication and writing skills; ability to transport students in agency vehicles.

Responsibilities

• Create and modify Pre-Employment Transition Skills (Pre-ETS) curriculum for students with IEP/504 plans.
• Deliver Pre-ETS curriculum in middle and high schools.
• Coordinate with teachers, Regional Director, and Vocational Rehabilitation Counselor about content and progress.
• Coordinate community assessment placements with schools, students/families, and business partners.
• Provide on-site support to help students learn tasks and integrate into the workplace; liaison with business personnel on needs.
• Analyze job tasks and environments to plan training and supervision needs.
• Observe and record client performance and complete assessment reports.
• Maintain ongoing communication with ORS Counselor, school personnel and family; submit reports timely.
• Other tasks as assigned by Administrator.
