Job summary

BayMark Health Services is seeking a Part-Time Compassionate Substance Use Counselor for an outpatient clinic in Colorado. The role includes counseling patients through MAT and coping strategies, conducting crisis management sessions, and collaborating with medical staff to support recovery goals.

The successful candidate will perform initial assessments, create treatment plans, involve families when possible, and develop discharge and aftercare plans.

Qualifications

• CAC II, CAC III, or LAC or LPC, LMFT, LCSW required.
• Minimum of two years of counseling experience preferred.
• Ability to develop comprehensive treatment plans and chart accurately.
• Excellent oral and written communication skills.

Responsibilities

• Provide individual counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment.
• Conducts initial assessment and evaluation interviews for service eligibility determination.
• Completes Bio-psychosocial assessment.
• Facilitates, establishes and reviews with each patient the initial treatment plan.
• Involve families and significant others in patient’s recovery process when possible.
• Respond to patient grievances and complaints.
• Develop aftercare plans and discharge plans.
• Other related duties as determined by supervisor.
