Job summary

Civita Care Center Cheshire in Cheshire, CT, is seeking a Speech-Language Pathologist (SLP) for a part-time role. You will evaluate residents with speech, language, cognitive-communication, and swallowing needs, and develop individualized treatment plans.

Join a collaborative rehabilitation team, provide skilled interventions, monitor progress, educate families, and maintain clear documentation. A CT license and a master's in Speech-Language Pathology are required; new grads encouraged to apply.

Qualifications

• Active Connecticut Speech-Language Pathologist license required.
• Graduate degree in Speech-Language Pathology from an accredited program.
• Strong clinical, communication, and organizational skills.
• New graduates are encouraged to apply.
• Experience in skilled nursing or post-acute care preferred, but not required.

Responsibilities

• Evaluate residents with speech, language, cognitive-communication, and swallowing disorders.
• Develop individualized treatment plans based on resident needs and functional goals.
• Provide skilled interventions addressing communication, cognition, and dysphagia.
• Monitor progress and adjust treatment approaches as appropriate.
• Educate residents, families, and caregivers on therapeutic strategies.
• Maintain clear and timely clinical documentation.
• Coordinate care with PT, OT, nursing, physicians, and other healthcare professionals.
