CentraCare Health Sauk Centre Hospital in Sauk Centre, MN, is seeking a compassionate Social Worker to join our interdisciplinary care team. You will assess patient and family needs and connect them with services, education and resources to navigate care successfully.

Using independent judgment and collaboration with providers, care coordinators and staff, you will advocate for patients, promote safety and proactively address barriers to care.

Qualificações

• Bachelor's degree in Social Work and Minnesota LSW license.
• Or Master's degree in Social Work and Minnesota LGSW licensure.
• Experience with diverse local communities preferred.

Responsabilidades

• Meet patients and families to identify needs and connect to resources.
• Provide education and emotional support for grief, illness and transitions.
• Advocate for patients and coordinate with the care team to address barriers.
