St. Mary Center in Lansdale, PA is hiring a part-time Social Worker LSW/CSW (MWF 8am-1pm). The role supports residents with emotional adjustment, completes social histories, and contributes to care planning and MDS processes. Overtime and double shift opportunities are available.

Qualifications

• Bachelor's Degree in Social Work from an accredited college is required.
• Must obtain a CSW and/or LSW certification.
• One to three years of professional experience and/or training, or an equivalent combination of education and experience.

Responsibilities

• Assist residents with emotional adjustment to the facility.
• Interview new residents and collect social history, assessments, and records as directed.
• Complete the social services portion of the MDS and care planning.
• Refer to psychological services as needed.
• Schedule and participate in interdisciplinary care plan meetings and morning rounds.
• Write summaries and progress notes as directed.
• Assist with pre-discharge evaluations and arranging Visiting Nursing Services.
• Assist with discharge planning and post-discharge follow-up.
• Develop professional relationships with residents and families.
• Educate residents/representatives on Advanced Directives.
• Adhere to Social Services Regulations.

Responsibilities for a social worker

• Assists residents with emotional adjustment to facility.

• As directed, interview new residents and records, social history, and assessments.

• Completes the social services portion of the MDS, Care planning, baseline, etc.

• Refers to psychological services as needed.

• Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.

• Writes summaries and progress notes as directed.

• Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.

• Assists with discharge planning. Follows up after discharge.

• Conducts required Cut Letters

• Writes discharge summaries as assigned.

• Develops a professional relationship with each resident and family.

• Educates residents/ representatives on Advanced Directives.

• Adheres to all Social Services Regulations.
