Nebraska Methodist Health System is seeking a social work professional to support discharge planning and patient/family education at Methodist Hospital in Omaha, NE. This part-time role focuses on coordinating post-acute care, resources, and transportation for patients.

Qualified candidates have a Bachelor’s degree or are actively enrolled in a Master of Social Work program with graduation within 24 months, plus at least six months of healthcare experience.

Formación

• Bachelor’s degree or active enrollment in an MSW program.
• Active MSW student with graduation planned within 24 months.
• Participation in mandatory in-services and continuing education as directed by supervisor.

Responsabilidades

• Assess patient/family needs and utilization of community resources (MOW, HHC, etc.).
• Coordinate post-acute care planning with healthcare team.
• Arrange transportation and notify care team of final discharge arrangements.
• Refer to Adult Protective Services (APS) or Child Protective Services (CPS) as appropriate.
