Qualifications

• High school diploma or equivalent required.
• Prior office, administrative, healthcare, or social services experience preferred.
• Long-term care or senior living experience preferred.
• Knowledge of HIPAA and resident rights preferred.

Responsibilities

• Support the Social Services Department with daily resident and family needs.
• Assist with new resident admissions, orientation, and paperwork completion.
• Prepare admission packets, forms, notices, and related documentation.
• Communicate with residents, families, and responsible parties regarding social service needs.
• Maintain resident files, social service records, and department tracking logs.
• Assist with MDS interviews and related documentation as assigned.
• Help coordinate meetings related to resident care, admissions, rehab, or discharge planning.
• Deliver required notices and resident communications.
• Assist with resident transition planning and follow-up as needed.
• Support department projects, education initiatives, and training programs.
• Collaborate with nursing, therapy, and other interdisciplinary team members to support resident well-being.
