Job summary

McCormick Post Acute is seeking a part-time Speech-Language Pathologist (SLP) to join our skilled nursing and rehabilitation team. You will evaluate residents, develop plans, and deliver therapy to improve communication and swallowing, while educating families and coordinating with the care team.

Qualifications

• Master’s or Doctorate in Speech-Language Pathology preferred.
• Current Speech-Language Pathologist license as required by state.
• CCC-SLP certification preferred.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop individualized treatment plans based on resident needs and goals.
• Provide skilled speech therapy services per physician orders and care plans.
• Educate residents, families and caregivers on therapy goals and techniques.
• Collaborate with interdisciplinary team to optimize patient outcomes.
• Complete timely documentation complying with regulatory requirements.
• Participate in care plan meetings and discharge planning as needed.
• Maintain compliance with all federal, state and facility guidelines.
