Job summary

Campbell Hall Rehabilitation Center in Campbell Hall, NY is seeking a part-time Speech-Language Pathologist (SLP) to assess, develop, and implement individualized treatment plans for residents with speech, language, cognitive, voice, and swallowing disorders.

You will collaborate with an experienced rehabilitation team, educate residents and families, contribute to care plans, and document progress to meet regulatory standards, while maintaining a flexible 20-hour schedule.

Qualifications

• Current New York State Speech-Language Pathologist license required.
• CCC-SLP preferred; Clinical Fellowship (CF) candidates encouraged to apply.
• Experience in skilled nursing, rehabilitation, or long-term care settings preferred.
• Knowledge of dysphagia management, cognitive rehabilitation, communication disorders, and PDPM preferred.
• Strong clinical, organizational, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Passion for delivering compassionate, resident-focused care.

Responsibilities

• Evaluate residents with communication, cognitive, speech, language, voice, and swallowing disorders.
• Develop individualized treatment plans based on clinical findings and physician orders.
• Provide skilled speech therapy services that support resident goals and functional outcomes.
• Perform swallowing assessments and collaborate with the interdisciplinary team regarding diet recommendations and dysphagia management.
• Educate residents, family members, and caregivers on treatment strategies and home programs.
• Document evaluations, treatments, progress notes, and discharge summaries in compliance with regulatory standards.
• Participate in care plan meetings, interdisciplinary rounds, and discharge planning.
• Communicate resident progress and recommendations to physicians, nursing staff, and rehabilitation colleagues.
• Maintain compliance with all federal, state, and facility guidelines.
