Job summary

United Community & Family Services, Inc. in Connecticut is seeking a part-time Client Support Specialist to provide therapeutic support, case management, education, and outreach to adult clients in Montville, Uncasville, and Oakdale through the Senior Center and Social Services.

The role helps connect individuals with community resources and services and supports care coordination. Open for 8 hours per week, with supervision and collaboration with other providers.

Qualifications

• Master's degree in M.A., M.F.T., or MSW required.
• CT state LMSW, LPC-A or MFTA license required.
• At least 2 years of case management/therapeutic support experience.

Responsibilities

• Provide case management and therapeutic support to adult clients.
• Facilitate education and outreach services.
• Liaise between individuals and community resources.
• Maintain documentation as per UCFS and Montville.
• Coordinate referrals to other levels of care; report abuse per guidelines.
• Participate in supervision and coordinate treatment with other providers.
• Collaborate with schools, DCF, courts, and other providers.
• Pursue licensure if not licensed.
