Job summary

HCA Florida Putnam Hospital in Palatka, FL is seeking a Part Time RN for Case Management to coordinate the care plan and evaluate medical necessity and level of care. You will work with an interdisciplinary team to support quality outcomes and efficient patient throughput in a collaborative hospital setting.

Required are Florida RN license or multi-state license, BLS within 30 days, and 3 years of acute care RN experience.

Qualifications

• Florida RN license and/or approved multi-state license is required.
• BLS-AHA Provider card required within 30 days of start.
• 3 years of RN experience in an acute care setting required.
• Prior experience in Case Management preferred.
• Certification in Case Management, Nursing or Utilization Review preferred.

Responsibilities

• Perform comprehensive psychosocial and medical assessments.
• Advocate for patients while leading the interdisciplinary team.
• Coordinate plan of care and ensure effective communications with physicians, patients and families.
• Conduct utilization management reviews and relay findings to payors.
• Promote patient throughput with focus on quality and efficiency.
