Job summary

Coffee Regional is seeking a part-time RN Case Manager (24-32 hours weekly) in Douglas, GA. This role coordinates clinically complex patient care across the continuum, ensuring quality outcomes and managing resources and services for patients.

The nurse must be licensed in Georgia and may hold a case management certification. Responsibilities include discharge planning support, collaboration with the health care team, and providing education and guidance to patients and families across the care
