What you’ll be doing:

1. Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
2. Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
3. Maintaining client records showing systematic assessments, planning interventions, and evaluations.
4. Initiating referrals as needed to other health team members.

1. Graduate of an accredited school of professional nursing.
2. Current license as a Registered Nurse.
3. Access to reliable transportation.
4. Maintains confidentiality in all aspects of the job.
