Job summary

Jobtailor is seeking a dedicated Registered Nurse to provide in-home skilled nursing services in Bergen County, NJ. You will assess clients, develop individualized care plans, supervise home health aides, and collaborate with physicians and therapists to ensure coordinated care.

Qualifications

• Active New Jersey Registered Nurse license - In good standing.
• Current CPR certification.
• Strong clinical assessment and critical-thinking skills.
• Home care, home health, geriatric, rehabilitation, or community nursing experience preferred.
• Experience supervising Certified Home Health Aides preferred.
• Knowledge of medication management, infection control, documentation, and care planning.
• Current negative TB test or chest X-ray.
• Ability to complete all required background checks and screenings.
• Excellent communication, organization, and customer-service skills.
• Compassionate, dependable, professional, and detail-oriented.
• Ability to work independently; travel throughout Bergen County; valid driver's license preferred.

Responsibilities

• Assess client needs and develop individualized Plans of Care.
• Provide skilled nursing services in client homes.
• Help ensure care meets professional, regulatory, and agency standards.
• Supervise and support Certified Home Health Aides.
• Collaborate with physicians, therapists, caregivers, and other healthcare professionals for coordinated care.
• Conduct initial and ongoing in-home nursing assessments.
• Provide direct nursing care based on client needs.
• Monitor clients and identify changes in condition.
• Communicate significant findings to families, physicians, and the care team.
• Complete accurate and timely clinical documentation.
• Coordinate services with healthcare professionals and caregivers.
• Conduct supervisory visits and evaluate care provided by home health aides.
• Provide education and clinical guidance to caregivers and families.
• Follow infection-control, safety, HIPAA, and agency requirements.
• Report incidents, concerns, and changes in client condition.
