Job summary

The New Jewish Home in Manhattan is seeking a part-time Registered Dietitian to join our interdisciplinary healthcare team. The selected candidate will provide nutrition assessment and clinical services to residents in long-term and sub-acute care units, with a 3-4 day work week and occasional weekend shifts.

You will document medical records including MDS 3.0, develop individualized nutrition care plans, educate residents and families, and collaborate with nursing, medical, and rehabilitation

Qualifications

• Registered Dietitian
• Experience in long-term and sub-acute care settings
• Ability to educate residents and families

Responsibilities

• Conduct nutrition assessments and provide clinical nutrition services for long-term and sub-acute care residents
• Complete accurate and timely medical record documentation, including MDS 3.0
• Develop, implement, and monitor individualized nutrition care plans
• Provide nutrition education to residents, patients, and family members as needed
• Participate in interdisciplinary care plan meetings
• Conduct meal rounds to ensure quality, safety, and resident satisfaction
• Collaborate with nursing, medical, and rehabilitation staff to support resident care goals
