Cherrydale Post Acute in Greenville, SC is seeking a compassionate Registered Dietitian (RD) for a part-time role. You will lead nutritional care planning, perform resident assessments, and collaborate with the interdisciplinary care team to support residents' health and quality of life.

Responsibilities include completing admissions assessments, updating care plans, monitoring nutrition status, and contributing to MDS dietary sections.

Qualifikationen

• Degree in Food and Nutrition, Dietetics, or related field.
• Completion of an accredited Dietetic Internship.
• Minimum of one year of clinical experience.
• Experience completing the dietary portion of MDS assessments.
• Skilled nursing or long-term care experience preferred.

Aufgaben

• Complete nutritional assessments for new admissions following federal and state guidelines.
• Perform quarterly, annual, and significant change nutritional reviews.
• Develop and update individualized nutrition care plans.
• Monitor nutritional status and identify residents at risk.
• Collaborate with Dietary Manager and interdisciplinary care team.
• Participate in care plan meetings and resident reviews.
• Provide recommendations on therapeutic diets, supplements, and nutrition interventions.
• Complete dietary portion of MDS assessments.
• Ensure compliance with federal, state, and facility nutrition standards.
• Maintain accurate and timely documentation.

Ausbildung

Degree in Food and Nutrition, Dietetics, or related field
