Job summary

Nashua Post Acute is hiring a part-time Registered Dietitian to provide individualized nutrition care for residents at our Nashua, NH facility. The RD will ensure medically appropriate diet plans and participate in discharge planning with the care team.

Responsibilities include developing nutrition care plans, reviewing dietary needs, maintaining quality assurance, and collaborating with interdisciplinary teams to achieve optimal health outcomes.

Qualifications

• Bachelor’s degree in Dietetics, Nutrition, or related field required.
• Current Registered Dietitian (RD) credential required.
• Minimum of 2 years of clinical experience preferred.
• Strong expertise in clinical nutrition, preferably geriatric or skilled nursing settings.
• Knowledge of applicable healthcare regulations governing nutrition in skilled nursing environments.

Responsibilities

• Assess residents’ dietary needs and support physician-ordered diet plans.
• Develop individualized nutrition care plans and regular/specialized diets.
• Assist in developing department food service procedures and purchases.
• Review surveys, implement corrective action plans, and report to the Administrator.
• Maintain ongoing quality assurance for the Food Services Department.
• Support discharge planning with nutrition components for residents.
• Monitor nutrition status and adjust plans to improve health outcomes.
• Collaborate with interdisciplinary teams to align nutrition with care goals.
