Job summary

Deer Meadows Home Health & Support Services seeks a Part-Time Physical Therapist Assistant (PTA) to provide in-home PT services throughout Philadelphia and surrounding counties. The PTA will work under the supervision of a licensed Physical Therapist to help patients regain mobility and independence.

Responsibilities include implementing treatment plans, educating patients and caregivers, documenting progress with Homebase EMR, and coordinating with the care team while adhering to regulations.

Qualifications

• Must hold PA PTA license.
• Graduate of an accredited PTA program.
• Home health experience preferred.
• Experience with Homebase Home Care EMR is a plus.
• Excellent communication, organization, and time management.
• Current CPR certification (or ability to obtain).
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled physical therapy treatments in patients' homes under the direction and supervision of a licensed Physical Therapist.
• Carry out individualized treatment plans designed to improve mobility, balance, strength, endurance, and functional independence.
• Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.
• Complete accurate and timely clinical documentation using Homebase Home Care EMR.
• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure high-quality patient care.
• Maintain compliance with agency policies and all applicable state and federal regulations.
