Job summary

Sigma Health Rehab in Brooklyn, NY is seeking a part-time Physical Therapist to join our skilled nursing facility team. You will evaluate residents, develop rehab plans, and help reduce pain through targeted exercises and hands-on therapy.

You will work with licensed therapists and support staff, enjoying mentorship, career growth, and flexible scheduling, including evenings and weekends. A current New York State PT license is required, with SNF experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.

Responsibilities

• Evaluate residents’ conditions & develop physical rehabilitation plans.
• Relieve residents’ pain using exercises, hands-on therapy & stretching.
• Help residents through their tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidence & protects facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements.
• Maintains a cooperative relationship among healthcare team members &contributes to team effort.
