Job summary

Sigma Health Rehab is actively seeking a part-time Physical Therapist (PT) to work in a Skilled Nursing Facility. The role focuses on evaluating residents, creating rehabilitation plans, and reducing pain through targeted therapy.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in a skilled nursing or geriatric care setting preferred; new grads encouraged.

Responsibilities

• Evaluate residents’ conditions and develop appropriate physical rehabilitation plans.
• Relieve residents’ pain using exercises, hands-on therapy and stretching.
• Help residents through their tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidence and protect the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements.
• Maintains a cooperative relationship among healthcare team members and contributes to team effort.
