Job summary

Westview Healthcare Center in Auburn, CA is seeking a part-time/per diem Physical Therapist to join our rehabilitation department. You will evaluate residents, create individualized plans, and help patients regain strength and independence.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active California Physical Therapist License or eligibility to apply.
• Familiarity with EHR systems such as Net Health or PointClickCare is a plus.

• Evaluating residents, creating individualized treatment plans, and celebrating patient progress.
• Maintaining accurate and timely documentation in accordance with facility guidelines and procedures.
• Helping residents regain strength, mobility, confidence, and independence.
• Working alongside a supportive interdisciplinary team that truly works together.
• Educating residents and families while building meaningful relationships.
• Bringing positive energy, professionalism, and compassion to work every day.
• Participating in the development and implementation of rehabilitation goals.
• Communicating with residents, families, physicians, nursing staff, and other members of the interdisciplinary team.
