Job summary

Qualifications

• Active Colorado Physical Therapist license or eligibility to obtain one.
• Graduate of an accredited Physical Therapy program.
• Experienced therapists and new graduates encouraged to apply.
• Strong clinical, communication, and patient-care skills.
• Reliable, professional and comfortable providing outpatient care.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized plans of care.
• Provide evidence-based outpatient physical therapy treatment.
• Monitor and document patient progress.
• Complete clinical documentation accurately and on time.
• Communicate with referring providers and other team members.
