Qualifications

• CA Physical Therapist License is required.
• Experience with in-house rehabilitation programs is a plus.

Responsibilities

• Assess patients' needs and develop tailored treatment plans based on conditions.
• Provide therapeutic interventions to help patients reach rehabilitation goals.
• Conduct discharge planning for transitions home or to next care level.
• Collaborate with multidisciplinary teams.
• Educate patients and families on daily living activities and techniques.
• Complete daily and weekly documentation of treatments and progress in a timely manner.

• Collaborate with multidisciplinary teams
• Educate patients and their families on techniques to improve daily living activities.
• Timely completion of daily and weekly documentation of patient treatments and progress
