Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to deliver primary and preventative care to residents in long-term care facilities. You will manage patients with acute changes, participate in advanced care planning, and coordinate with the interdisciplinary team.

The role requires Ohio NP licensure, board certification, DEA licensure, and experience in geriatrics or adult care within LTC settings. EMR proficiency and collaboration with primary care physicians are essential.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification as an NP.
• DEA licensure required.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care setting.
• Ability to work with an EMR system.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide primary and preventative care for long-term care patients in skilled nursing facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.
