Job summary

CareMed is seeking a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities. You will manage acute changes in condition and participate in advanced care planning and chronic care management.

You will prescribe medications per KY law, order and review treatments, and collaborate with primary care physicians as part of an interdisciplinary team. DEA licensure and board certification are required.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board certification as NP.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care settings.
• Experience working with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work in an interdisciplinary care team.
• Psych experience a plus.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.




• Manage Residents with acute change in condition.




• Advanced care planning and chronic care management.




• Prescribe appropriate medications, including controlled substances in accordance with KY Law.




• Order, review and/or follow-up with treatments and diagnostic testing.




• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.




• and/or the care plan.




• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
