Job summary
Riveon is seeking a part-time Primary Care Nurse Practitioner to provide comprehensive, patient-centered care within an integrated behavioral health and primary care setting in Ohio. The NP will assess, diagnose, treat, and monitor acute and chronic conditions while promoting wellness and prevention.
The role includes medication management, counseling, collaboration with a multidisciplinary team, and documentation in the EHR.
Qualifications
- A Master’s degree in Nursing and NP certification with prescriptive authority.
- Ohio Board of Nursing registration with prescriptive authority is required.
- Up-to-date CPR certification and professional communications skills.
Responsibilities
- Take medical histories, perform physicals, and assess social and substance use histories.
- Provide counseling, motivational interviewing, and behavioral health with medication management.
- Diagnose and treat acute and chronic primary care conditions.
- Monitor treatment plans, labs, adherence, and potential adverse effects.
- Promote preventive care, screenings, immunizations, and wellness maintenance.
- Document in the EHR and coordinate care with the multidisciplinary team.
