Job summary

Riveon Mental Health and Recovery is seeking a part-time Primary Care Nurse Practitioner to deliver patient-centered care across the lifespan in an integrated behavioral health and primary care setting.

Responsibilities include comprehensive medical assessments, counseling, and medication management, with collaboration within a multidisciplinary team to support recovery from substance use disorders. Ohio prescriptive authority and NP certification required.

Qualifications

• A Master's degree in Nursing and certification as an Advanced Practice Nurse with prescriptive authority.
• Certification as a Nurse Practitioner is required.
• CPR certification.
• Excellent oral and written communications skills.
• Familiarity with Windows and Microsoft Office.

Responsibilities

• Conduct comprehensive medical histories, physicals, and assessments including substance use history.
• Provide counseling, motivational interviewing, and behavioral health interventions alongside medication management.
• Diagnose and treat acute illnesses and chronic conditions in primary care.
• Manage chronic conditions, monitor treatment effectiveness, and review labs/medications.
• Promote preventive care, screenings, immunizations, and wellness.
• Integrate medical treatment plans into the EHR and coordinate care with the team.

POSITION PURPOSE AND OBJECTIVES

The NP is responsible for assessing, diagnosing, treating, and monitoring acute and chronic medical conditions while promoting disease prevention, health maintenance, wellness, and improved overall health outcomes. This may include providing services to individuals with opioid use disorders and other substance use disorders, combining medication management with counseling and behavioral therapies to support recovery.

POSITION PURPOSE AND OBJECTIVES

The NP is responsible for assessing, diagnosing, treating, and monitoring acute and chronic medical conditions while promoting disease prevention, health maintenance, wellness, and improved overall health outcomes. This may include providing services to individuals with opioid use disorders and other substance use disorders, combining medication management with counseling and behavioral therapies to support recovery.

Essential Job Functions

• Conduct comprehensive medical health histories, physical examinations, and medical assessments including social and substance use history assessments, screen for withdrawal symptoms.
• Offer counseling, motivational interviewing, and behavioral health interventions alongside medication management.
• Diagnose and treat acute illness, chronic medical conditions, and common primary care concerns.
• Manage chronic health conditions and monitor treatment effectiveness, medication adherence, laboratory results, and potential adverse effects.
• Promote Prevention and wellness through preventative care, health screenings, immunizations, and routine health maintenance.
• Integrate physician and behavioral health can collaborate with the treatment team to address client's whole-person health needs.
• Provide medication management with a scope of practice.
• Coordinate Care and Referrals
• Incorporates the individualized medical treatment plan into the existing Electronic Health Record (EHR).
• Educate and engage clients regarding diagnosis, treatment options, medications, preventative care and self-management
• Maintains accurate Clinical documentation
• Promote client safety and quality by participating in quality improvement activities and utilize evidence-based practices.
• Collaborate as interdisciplinary team member
• Demonstrates professional accountability by maintaining required licensure and adhere to organizational polices.
• Conducts Peer Review activities as required.
• Attends Clinical and Agency meetings as required.
• Meets regularly with collaborating Physician to effectively communicate about cases.
• Achieves required productivity expectation.
• Reacts to change and stress productively.
• Maintains regular and predictable attendance and punctuality.
• Gets along with others and works as a team.

Requirements

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

• In order to perform the essential functions of this job, after an orientation period, the employee must possess the following: a working knowledge of Riveon Policy and Procedures, including sensitivity and adherence to clients' rights, confidentiality, and health and safety issues.
• The employee must exhibit sensitivity to different cultures.
• This employee must be up to date on all certifications, continuing education, and registration with the Ohio Board of Nursing which allows prescription authority under supervision in the State of Ohio.

WORKING CONDITIONS

• The work is mainly provided in an office setting, although travel may be required dependent upon services contracted through other agencies such as medical, rehabilitation, or correctional settings. Because of the variety of environments and physical conditions that will be encountered, this individual must be able to successfully interact and navigate in all types of facilities and weather conditions.
• This individual may periodically be required to lift and/or carry up to 20 pounds.
• Must be able to physically interact with client and others in emergency situations to prevent harm to client and others.
• The individual in this position must be able to communicate clearly both verbally and in writing.
• Due to the extensive computer work this individual must be able to sit for extended periods of time and operate required equipment.

Requirements/Qualifications
