Job summary

Trumen Physicians + Associates is seeking a part-time Post-Acute Care physician to join our post-acute team in Wharton, TX. You will manage a patient panel in skilled nursing and rehabilitation facilities, focusing on reducing readmissions and guiding transitions of care.

Ideal candidates hold an MD or DO with Texas license, board eligibility or certification in Internal Medicine, Family Medicine, or Geriatric Medicine, and have 1+ year of SNF/post-acute experience.

Qualifications

• MD or DO with a current, unrestricted Texas medical license.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• 1+ year of experience in post-acute, geriatric, or SNF care preferred.
• Comfortable practicing independently in skilled nursing facilities.
• Strong clinical judgment, communication skills, and a patient-centered mindset.

Responsibilities

• Provide in-person medical care to patients in assigned post-acute facilities.
• Perform assessments, manage treatment plans, and update documentation in the EMR.
• Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care.
• Participate in Trumen’s value-based care and transitional care programs.
• Develop strong working relationships with facility staff and leadership.
• Serve as a liaison between Trumen and facility teams.
• Be available in assigned facilities anytime during regular business hours on scheduled days.
