Job summary

Care Partners Home Health, a part of LHC Group, is seeking a Physical Therapist Assistant for part-time work delivering in-home therapy services across Wetzel, Tyler, Marshall counties. You will follow a plan of care, instruct patients in exercises, and coordinate with the supervising PT and nursing team to support patient well-being.

Requirements include current PTA license in state of practice and a valid driver’s license with reliable transportation; CPR certification preferred.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification is preferred or can be completed within 90 days of hire.

Responsibilities

• Follow state laws and the defined plan of care as supervised by a PT.
• Instructs patients in active and passive exercise, gait, ADL, transfer, safety, and prosthetic training.
• Observe and report patient responses, coordinate with the healthcare team.
• Educate patient, family, and caregivers on therapy within scope.
