STAT Home Health is hiring a PRN Physical Therapist in Natchitoches, LA to evaluate patients, develop plans of care, and provide skilled PT services in home settings. You’ll work with a care team to support functional goals and ensure quality outcomes for patients in their homes.

Eligible candidates are licensed in Louisiana with at least one year of PT experience, CPR certified, and excellent time management. Home health experience is preferred, and a company ESOP offers ownership benefits.

Qualificações

• Licensed by the Louisiana Physical Therapy Board
• Minimum 1 year of PT experience
• CPR certification required
• Excellent time management and patient care
• Reliable transportation to patient homes; home health experience preferred

Responsabilidades

• Assess patient function and assist in care plans
• Deliver PT services per scope of practice and plan of care
• Observe, document patient response and report changes to physician/nurse
• Prepare a written summary to the physician every 60 days
• Help set rehab goals and guide therapeutic activities
