Job summary

Enhabit Home Health & Hospice in the United States is hiring a part-time Physical Therapist to support home-based patients. You will assess mobility, design treatment plans, and use modalities to improve functional independence under physician orders and the branch director's supervision.

The role focuses on chronic care and rehabilitation goals, with a 24-hour work week spread over four days and weekend coverage Friday through Sunday. A valid state license and driver's license are required.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Improve mobility and functional independence through therapy.
• Administer skilled care per physician orders and established plan of care.
• Educate family and health care team; work under supervision of Branch Director.
