Job summary

Enhabit Home Health & Hospice is seeking a part-time Physical Therapist to provide skilled care at patients’ homes. The role focuses on improving mobility and independence, following physician orders and the established plan of care. Work 24 hours per week over four days with the support of a Branch Director.

You will support families, coordinate with the care team, and contribute to a patient-centered home health program with comprehensive benefits and ongoing professional development.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation.
• Must be able to drive in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and daily living skills.
• Administer skilled care per physician orders under the supervision of the Branch Director.
• Teach patients and family members as part of care plans.
