Job summary

Spring Mill Health Campus in Indiana seeks a compassionate Part-Time Physical Therapist to join our rehabilitation team and help residents improve strength, mobility, independence, and quality of life. The PT will evaluate mobility, develop treatment plans, provide exercises, monitor progress, document care, and collaborate with physicians and nursing staff in a supportive, team-focused environment.

Prior long-term care or SNF experience is preferred but not required; licensure in Indiana is

Qualifications

• Must hold a current Physical Therapist license in Indiana.
• Graduate of an accredited physical therapy program.
• Strong communication, documentation, and clinical skills.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional abilities.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, mobility training, and rehabilitation services.
• Monitor residents’ progress and adjust treatment plans as needed.
• Maintain accurate and timely clinical documentation.
• Educate residents, families, and team members on treatment goals and safety.
• Collaborate with physicians, nursing staff, and the interdisciplinary care team.
• Follow all professional, facility, and regulatory standards.

Spring Mill Health Campus is seeking a dedicated Physical Therapist to join our rehabilitation team and help residents improve their strength, mobility, independence, and overall quality of life.
