Job summary

A Physical Therapist is sought to provide intermittent PT services in residents' homes, therapy clinics or living facilities in Missouri. The PT will collaborate with an outpatient/director and the interdisciplinary team to deliver prescribed therapy and monitor patient progress.

Qualifications include a BS in Physical Therapy, Missouri license, NBCOT certification, and two years of experience preferred. Part-time or PRN schedule; PTO accrual and mileage reimbursement offered.

Qualifications

• Graduate of an accredited Physical Therapy program (minimum BS degree).
• Current NBCOT certification and valid Missouri therapy license.

Responsibilities

• Assess patient functional ability and potential.
• Complete PT evaluation and establish an effective plan of treatment.
• Develop and implement medically prescribed PT plan of treatment.
• Document visits accurately and timely.
• Coordinate treatment with interdisciplinary team and supervise PTA and aides as assigned.
• Instruct patients and caregivers in PT activities and home exercises.
• Attend case conferences and agency in-service programs.
