Job summary

HealthPro Heritage is seeking a Part-time Physical Therapist to join our assisted living community in Holmdel, NJ. This role offers a weekday schedule with no evenings or weekends, and a supportive, team-oriented environment that values patient-centered care.

You will assess mobility and function, design and implement therapy plans, educate patients and families, and collaborate with other clinicians to optimize outcomes across nursing facilities, retirement communities, hospitals, and home care

Qualifications

• Degree in Physical Therapy from an accredited program.
• Valid state licensure as a Physical Therapist, or licensure eligible.
• Strong clinical assessment, diagnostic, and patient education skills.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, mobility, strength and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy and educate patients/families.
• Documentation: Maintain accurate records of evaluations, progress, and plans.
• Collaboration: Work with healthcare providers and participate in multidisciplinary meetings.
