ABUNDANT HOME HEALTH LLC in Arlington, TX seeks a Part-Time Physical Therapist to plan, implement, and evaluate patient care in home health and facility settings. The therapist will coordinate care with physicians and family, maintain documentation, and ensure compliance with infection control standards.

Requires a BS in Physical Therapy, state licensure, CPR, and 1 year of relevant experience preferred. Travel to patient locations within the service area may be required.

Qualifications

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
• Current State Licensure as a Physical Therapist or meets requirements in 42 CFR §484.115(h).
• One year experience as a Physical Therapist in acute care/rehabilitation/home health preferred.
• Therapy skills per generally accepted standards; good interpersonal skills; CPR proof; knowledge of durable medical equipment.

Responsibilities

• Performs ongoing clinical assessment, including OASIS where applicable, to identify patient needs and determine level of functioning.
• Develops and evaluates the plan of care in collaboration with patient, family, and caregiver(s).
• Documents, completes clinical notes, and revises the Plan of Care based on evaluations and assessments.
• Provides services ordered by physicians, oversees team members when applicable, and aligns with patient needs.
• Applies infection control concepts and standard precautions in patient care activities.
