Job summary

Enhance Rehabilitation in Kenosha, WI is seeking qualified Physical Therapists to join our dynamic SNF team. You will evaluate and treat patients following an individualized plan of care, ensuring high-quality outcomes and patient satisfaction.

You will collaborate with facility staff, supervise assistants, and document progress and outcomes to drive continuous improvement in care delivery.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
