Job summary

Continuum Therapy Partners, LLC is seeking a licensed Physical Therapist for a part-time daytime role at Colonial Health Services in Colby, Wisconsin. The ideal candidate will evaluate and implement individualized treatment plans, coordinate with other disciplines, and educate patients and families on home exercise programs for injury prevention.

Requirements include a Doctor of Physical Therapy (DPT) degree, an active state license, and NPTE completion.

Qualifications

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
