Job summary

BMS Family Health and Wellness Centers in Brooklyn, NY seeks a licensed Physical Therapist for a part-time, exempt position. The PT will evaluate patients, create individualized treatment plans, and deliver evidence-based therapies in an outpatient community health setting.

Qualifications

• DPT or master's in physical therapy from an accredited program.
• Current, unrestricted New York State Physical Therapy license.
• CPR/BLS certification and strong communication skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on findings and goals.
• Provide therapeutic interventions including exercise, manual therapy, gait training, and education.
• Monitor progress and modify treatment plans as needed.
• Maintain accurate clinical documentation and coordinate care with teams.
