Job summary

ABL Health Care in Oakland, CA is seeking a compassionate Part-Time Physical Therapist Assistant (PTA) to join our Home Health team. You will evaluate patients, develop treatment plans, and help improve mobility and quality of life while collaborating with a clinical director and staff.

This role offers dedicated clinical support and a benefits package including health, dental, vision, 401(k), life insurance, and paid time off.

Qualifications

• Current Physical Therapist Assistant (PTA) License to practice in California - Required
• Experience in Home Health or Hospice care - 1 Year Required
• Current CA Driver’s License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
