Job Summary: The Physical Therapist will screen, evaluate, re-evaluate, discharge, and revise care plans as supervised by a physician.

Qualifications:

• Bachelor's, Master’s, or Doctorate in physical therapy from an accredited program.
• Active state license as required.
• Certification and licensure per state and national requirements.
• Excellent organizational, time management, written, and verbal communication skills.
• Knowledge of Functional Pathways policies, procedures, and clinical programs.
• Ability to supervise Physical Therapy Assistants.
• Completion of all required screenings, certifications, and licensure.
