Qualifications

Responsibilities

• Review orders, assess regimens, and optimize therapy.
• Counsel patients and families on proper use and adherence.
• Collaborate with physicians and nurses to resolve medication issues.
• Participate in medication safety, quality improvement, and audits.
• Develop and implement clinical protocols and guidelines.
• Monitor outcomes and adjust therapy for safety and efficacy.
• Document interventions and MTM activities in the EHR.
• Maintain inventory and monitor stock levels; identify cost-saving opportunities.
• Stay current with pharmacotherapy advancements.
