PharMerica in Sand Springs, OK is seeking a part-time Pharmacist for our long-term care, closed-door pharmacy team. You will review orders, provide clinical recommendations, and maintain resident profiles while ensuring compliance and patient safety. Varying days, 16–24 hours per week.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles including medical history, labs, and current medications.
• Ensure laboratory tests for monitoring high-risk medications are ordered, reviewed, and communicated to the team.
• Collaborate with physicians, nurses, and others to provide clinical and technical support.
• Document all communications and clinical interventions in the patient’s medical record.
• Implement and enforce policies for safe handling, dispensing, and oversight of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice.
• Report medication-related errors to the Pharmacy Manager.
• Educate clinical team and staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
