Job Description

Position Overview

• Collaborate with physicians, Nurse Practitioners, and staff in the Skilled Nursing Facility.
• Manage care for cardiac diseases such as atherosclerotic heart disease, heart failure, arrhythmias, and more.
• Develop and monitor treatment plans.
• Provide consulting services and off-hours emergency coverage.
• Interpret diagnostic procedures like ECGs, echocardiograms, and ultrasounds.

Essential Functions

• Interact professionally with patients, families, and staff, embodying the organization’s ICARE values.
• Accurately document care in medical records.
• Participate in interdisciplinary team meetings and patient education.
• Communicate effectively and compassionately with patients and families.
