Responsibilities

• Provide intensive peer support through a personalized mix of social, recreational, skill teaching and linkage development for individual matches.
• Maintain weekly contact with each match and initiate discussions about skills and supports during meetings.
• Facilitate peer/mutual support meetings in day programs and residential settings; complete meeting reports and sign-in sheets.
• Perform travel and reporting responsibilities for match relationships; maintain timely documentation for all services; participate in bi-weekly team meetings and trainings.
