Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (example: PCS).
• Bilingual (English/Spanish).

Responsabilités

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patient needs.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with caregivers.
• Collaborate with other providers to coordinate care in patients' medical neighborhoods.
• Complete a minimum number of patient visits weekly as required by the agency.
• Maintain patient charts with progress updates, logs, and required documentation.
• Meet with agency personnel to ensure understanding of expectations and standards for high-quality care.
• Adhere to agency policies, procedures, and applicable laws.
