Job summary

Medley Health is seeking a Physician or Nurse Practitioner for part-time, on-site pain management across assisted living facilities in New York. You’ll evaluate and manage pain, perform injections, and coordinate with interdisciplinary teams to optimize resident comfort with a focus on safety and autonomy.

The role emphasizes independent practice, strong documentation, and collaboration, with malpractice coverage and supportive administration to balance clinical and schedule needs.

Qualifications

• MD or DO with pain management experience and active NY license.
• NP with pain management experience and active NY NP license.
• Experience with joint or trigger-point injections and autonomous practice.

Responsibilities

• Perform comprehensive pain management evaluations and follow-ups in assisted living.
• Diagnose and manage acute and chronic pain in older adults.
• Prescribe and manage medications, including controlled substances, per regulations.
• Perform interventional procedures such as joint and trigger-point injections.
• Develop individualized treatment plans focused on safe, effective pain control.
• Document assessments and progress notes accurately and timely.
• Communicate recommendations to facility teams and support care transitions.
