Medley Health is seeking a Physician or Nurse Practitioner for part-time, on-site pain management across assisted living facilities in New York. You’ll evaluate and manage pain, perform injections, and coordinate with interdisciplinary teams to optimize resident comfort with a focus on safety and autonomy.

The role emphasizes independent practice, strong documentation, and collaboration, with malpractice coverage and supportive administration to balance clinical and schedule needs.

Qualifications

• MD or DO with pain management experience and active NY license.
• NP with pain management experience and active NY NP license.
• Experience with joint or trigger-point injections and autonomous practice.

Responsibilities

• Perform comprehensive pain management evaluations and follow-ups in assisted living.
• Diagnose and manage acute and chronic pain in older adults.
• Prescribe and manage medications, including controlled substances, per regulations.
• Perform interventional procedures such as joint and trigger-point injections.
• Develop individualized treatment plans focused on safe, effective pain control.
• Document assessments and progress notes accurately and timely.
• Communicate recommendations to facility teams and support care transitions.

• MD/DO: Competitive, based on experience and schedule (discussed at interview)

• NP: Competitive, based on experience and autonomy (discussed at interview)

About the Role

Medley Health is looking for a compassionate Physician or Nurse Practitioner for a part-time opportunity providing on-site pain management services across assisted living facilities. You’ll work independently with strong administrative support, collaborating with interdisciplinary teams to manage residents' pain through medication management and interventional procedures.

What You’ll Do

• Perform comprehensive pain management evaluations and follow-up visits in assisted living settings

• Diagnose and manage acute and chronic pain conditions in older adult populations

• Prescribe and manage medications, including controlled substances, in accordance with state and federal regulations

• Perform in-office interventional procedures such as joint injections and trigger-point injections

• Develop individualized treatment plans focused on safe, effective pain control

• Document assessments and progress notes accurately and timely

• Communicate recommendations to facility clinical teams and assist with care transitions as needed

Qualifications - Physician (MD/DO)

• MD or DO with experience in pain management; Board Certification/Eligibility in Pain Management, Anesthesiology, PM&R, or a related field preferred

• Active New York medical license

• Active DEA license required

• Experience performing joint and/or trigger-point injections

• Experience in assisted living, geriatric care, or related settings preferred

• Strong clinical judgment, documentation, and communication skills

Qualifications - Nurse Practitioner

• NP license with pain management experience (Adult-Gerontology preferred; FNP considered with relevant experience)

• Active New York NP license

• Active DEA license required

• Experience performing joint and/or trigger-point injections

• Experience in assisted living, geriatrics, orthopedics, or internal medicine preferred

• Ability to work autonomously and manage a patient panel

• Strong documentation and communication skills
