Jefferson Health Northeast in Philadelphia, PA seeks a part-time Physical Therapist I responsible for evaluation, treatment, and discharge planning for patients across acute care, skilled facilities, acute rehab, and outpatient settings. The role emphasizes coordination with a multidisciplinary team and adherence to the hospital’s mission and values.

The candidate must have a PA-licensed PT (Bachelor/Master/Doctorate in PT), and BLS certification, with lifting capacity of 40–50 lbs and ability

Formación

• Bachelor, master, or doctorate in physical therapy from an accredited program.
• Licensed Physical Therapist in Pennsylvania (PT).
• Basic Life Support (BLS) certification.

Responsabilidades

• Provide therapy evaluations, treatment planning, and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, and outpatient settings.
• Participate in student education and supervision as a part-time or full-time clinician.
• Plan, organize and deliver treatment programs to help clients recover and rehabilitate.
• Document health progress, therapies performed, progress notes and discharge summaries.
• Collaborate with co-workers and other staff to coordinate treatment plans.
